how does SQP within fmincon calculate constraints violation or relative one ?

i'm minimizing the objective but the relative max constraints violation is always 0 which it shouldn't be.nothing about how it's given can be found in help file.what's more, the solution given by fmincon have really small improvements than initial points . where can the problem be ?
