The complete code for the situation you describe is,

It has been proven (Lorentz, Einstein) that it is not possible to synchronize clocks if at least one of the clocks is moving. If clocks cannot be synchronized, then position cannot be measured correctly, and so accurate tracking is not possible.

You do not need any code. Simply give a sheet of paper and a pen to each patient and ask them to write down where they are.
A more ecological method (paper is killed trees) is to ask the NSA for the tracking data of the patient's smart phones. Why not using it, when it is already recorded?

Have the patient say "I am here" over and over again. Use triangulation on the sound waves to figure out where the sound originated from at each time-point, if you answers in 2 dimensions; add a third mic off-plane if you want the answers in 3 dimensions.

What if the patient doesn't have a smart phone? Gotta consider the edge cases...

NASA tracks even the dumb phones. The cell towers track location in order to direct signals properly.
