How to wavelet decomposition for following problem?

I want to write a code for DB4 wavelet decomposition. I have signal size is 65x1600 (EEG signal)and want to split into 5 decomposition level D1-D5, A5 with frequency band A5=0-4 hz D5=4-8 hz D4=8-14hz D3= 14-32hz D2-32-64 hz and D1 noises..
Kindly help me out how can i split?
