How to get variable values from user using a pop-up in a table form ?

I want to take some values from user . I want it to be an interactive way. Like more than one vlaue can be feed at a time for example. To get the name, age, salary, and gender. I want a popup when i run the code and in that pop-up window I want user to enter the value of name age salary gender in front of their box/column . It might be simple one but I am learnig to do this stuff on matlab . Can anyone help me with this?

Here is an example snippet that you can adapt to add more items. You can get strings too - just don't call str2double().
