C++ model - connect to Matlab/Simulink

Hi, My colleague has developed a stand alone model in C++. At the moment parameters entered via dialog and results written to text file. Would like to integrate this model in Matlab/Simulink to facilitate automated runs and direct plotting of results.

Any suggestions on what to research as possible ways to do this?
