combine MPC with neural network

I built my plant model using Neural network toolbox Fitting Tool. Now I want to control my MIMO plant using MPC, but MPC requires the plant to be a transfer function or state-space function.

So How can I transform my NN plant model into such functions?

Or is there other way I can use the NN model in the MPC control system?
