I would recommend checking the basic relationship between FM (Frequency Modulation) and PM (Phase Modulation) signal equations. There are many useful textbooks and web pages, like this material .
In your case, the output signal s(t) is equivalent to A_c[2pi(f_c + k_f*w(t))t].
