Is it possible to control a physical process with simulink?

Hello.
The problem I have is that I need to control an outside process in a closed loop with Simulink. So the controller itself must be created in Simulink, go ouside to the process (for example a DC motor) and I need to receive feedback from the output of the process. Is it possible to communicate in such a manor with Simulink so that the program could manipulate outside signals, and if yes, does anyone have a clue how?
