How to code a 3 input with 1 ouput neural network in AppDesigner?

Background: It has been identified that a Neural Network of 3 inputs, 1 target, 500 hidden neurons, with a dataset of 272x3 is giving a relatively consistent good result. Weights and biases have been identified. The code for this neural network system has been attached.

Question: How to modify the code in order to works in AppDesigner as a callback?
