Create a value map from input signal in a Simulink block

Example:

Temperature 500 630 700 870 900 950

Output 5 4 2.8 2 1 0.5

The output from a temperature signal ranges from 500 to 950, depending on the temperature, the corresponding Output value should be sent to the next block. How can this be mapped, what block can be used for the same?
